The Opioid Alternative Program was designed to offer more alternatives to manage their pain. This program allows those who are prescribed Opioids or ones that can be prescribed opioids to instead treat their pain through the use of medical cannabis.

How to Know if I qualify for the Program

It may seem to good to be true that you are slightly hesistant to apply because you are just not sure if you would qualify for this program. Here's a list so you can be sure that you qualify for the Opioid Alternative Program:


  • Must be 21 years old or older
  • Has a current prescription for an Opioid
  • Could be prescribed an Opioid based on health condition
  • Does not have a Commercial Driver's Licence 
  • Does not have a school bus permit



Be advised that there are no caregiver options for the Opioid Alternative Program and you cannot change your dispensary during the 90 registration period.

The Registration Process

While it might sound so nice to be pain free using a natural medicine like medical cannabis, some might not know how to register. So here's a step-by-step process of everything you need to do in order to register for your Opioid Alternative Card!


  • See a Physician
    • A Physician will have to fill out a certificate using the Illinois Cannabis Tracking System (Don't worry, it's confidential). 
  • Before or after the Physician certificate, you will have to create an account in order to register for your card online. You can start your registration here:  https://etk.icts.illinois.gov 
  • After you create your account, you will need to fill out the registration form. Within the registration form, be prepared to have the following list of items:
    • Passport like photo
    • Driver's licence or state ID
    • Proof of Illinois residency
    • $10 payment for the registration card
  • When all the information is filled out, and they receive your payment, you will receive an email with their electronic medical cannabis registry card

And that's it! The process is done! As soon as you receive the electronic medical cannabis registry card, you are allowed to purchase cannabis from the dispensary that you chose! Remember to bring your state ID or driver's licence. It's important to note that you will have to pay a $10 fee every 90 days.

General Provisions

List of Provisions to Keep in Mind

  • All registered patients are limited to 2.5 ounces of medical cannabis per 14 day period
    • There are no exceptions


  • You can't be registered in the Medical Cannabis program and the Opioid Alternative Program at the same time
    • Patients with a qualifying condition may apply for the MCPP card 


  • After your 90 period is over, your card is no longer valid
    • You may register again with the submission of another physician certification

Opioid Alternative vs. Medical Cannabis Program

Physician Certification

  • Medical Cannabis Pilot Program (MCPP) 
    • The paper certification document is provided to the patient by the doctor
  • Opioid Alternative Pilot Program (OAPP)
    • The physician will submit the certification electronically through the secure Illinois Cannabis Tracking System

Registration Periods

  • MCPP
    • Patient can register for a 1, 2, or 3 year period all at varying costs
  • OAPP
    • The registration period is limited to 90 days

Registration Card Fees

    • $100 for one year registration card
  • OAPP 
    • $10 for a 90 day period
