What is the Pilot Program?

The Medical Cannabis Pilot Program allows those with qualifying conditions to naturally medicate through the use of cannabis.

How long does my medical card last?

When you register for your medical card, you have the option to register for a 1 year, 2 year, or 3 year card. When your card expires, you are able to renew your card again.

Medical Cannabis Provisional Card

Here's all you need to know about the Provisional card access:


  • The Provisional Card only lasts for 90 days while your application is being processed
  • You must select a dispensary to use for 90 days
  • You will receive your provisional access to your dispensary the following day via email


It's important to note that the only way to receive the provisional card is to apply online. Those that apply through a paper application cannot receive the provisional card.

What Can you expect when applying for your card?

The application process may be quite confusing and you may be unsure of how to proceed, here is what you can expect:


 

To apply for the provisional access card for the MCPP (Medical Cannabis Pilot Program): Start the registration process@  https://medicalcannabispatients.illinois.gov



Then we will upload your Physician certification after the Clinic visit, you can finish the process and print out your provisional access card.


When filling out your application, you can start the process on your own using the link above. Make sure you remember your username and password! When you start your application at home, it will make it easier for us to help you and become a faster process. 


When you come into the office to register and get a doctor certification, make sure you please bring the following:


  •  Illinois Driver’s License or State I.D. Color copy 
  •  2 Current Proofs of Residency, such as an Electric bill, bank statement, etc. 
  • If you are on Disability: Benefit Verification Letter (website: http://ssa.gov/myaccount/)
  •  If you are a Veteran: DD214 Discharge papers, bypass doctor fee. (12 mo patient in the VA)

MCPP- Doctor visit- $300.00 cash.


OAPP - Doctor Visitor - $150.00 cash. 


OAPP Provisional card-$10.00 (IDPH fee due every 90 days)


Fee’ s for Patients Medical License (IDPH)

  • 1 year- $100
  • 2 years- $200
  • 3 years- $250

Caregiver Fees

  • 1 year- $25, 
  • 2 years- $50, 
  • 3 years- $75

Fee’ s for Veterans and SSI/SSDI

  • 1 year- $50 
  • 2 years- $100 
  • 3 years- $125.00
